Did you know that your child can be considered a New Patient if we have not seen him/her for 3 years. We follow the American Academy of Pediatric (AAP) guidelines. Your child needs to be current on their well checks in order for us to process any referrals, forms or prescriptions. Well checks are recommended at 2mo, 4mo, 6mo, 9mo,1 yr, 15 mo or 18mo, 2 yr and once a year after that. We also follow the AAP guidelines for vaccines. We understand that you may want to separate or not give your child the recommended vaccines. Therefore, we do require the parent to sign a refusal to vaccinate if you choose not to give the recommended vaccines, otherwise we will not be able to continue seeing your child.

Saturday Appointments

All office visits are by appointment only. On Saturdays we offer same day sick, emergency visits and scheduled well visits. Unfortunately, if we do not receive a 1 hour notice to cancel or re-schedule we will not be able to schedule your child on Saturdays in the future.

Sick Appointments

Most minor illnesses and injuries can be managed by the parent. Many common illnesses and injuries are covered in our handouts and child care books. Please refer to them before calling the office. Prolonged illnesses and complicated questions require an appointment with a physician.  It is understandable that you may develop a preference for a provider, and we will always try to schedule your appointments with that in mind. However, when it is not possible, we ask for your understanding and request that you make an appointment with one of the available providers.

Please make an appointment for each child that needs to be examined by a provider. If you do request for us to look at an additional child, not on the schedule, we will do our best to find the next available appointment and there may be a wait.

For your convenience, we have two separate waiting areas. Please use the sick area if your child, or any child with you, has symptoms of illness such as congestion, cough, diarrhea, earache, mattery eyes, fever, headache, infected skin, nausea, oozing sores, rashes, runny nose, stomach ache or vomiting.

Please sign in on the sick side and keep all children with you in that area until you are called by our medical staff.

Appointment Cancellations

If you are unable to keep an appointment, please call the office ONE hour in advance of your appointment so we may use that time for another child. There could be another family with sick children that would benefit from an earlier appointment time. A $25.00 charge will be assessed for missed appointments, or those not canceled ONE hour prior to the appointment time. Consultations, which are are extended appointments, require a 24 hour cancellation notification; otherwise a $50.00 fee will be assessed to your account. Unfortunately, if you do not cancel your Saturday apt 1 hour prior to the appointment time we will not be able to schedule Saturday appointments in the future.

A Parent or Guardian Must Accompany Minors

A parent or legal guardian must accompany the child to each visit. A signed consent form must be on file for any other adult, over the age of 18 years, other than the parent or legal guardian, to bring the child in.  You will be required to update this form yearly.

A parent or legal guardian must accompany every child for their 1st visit.

Shot Appointments/ Nurse Visits
