New Patient Portal

All patients will need to register for the new portal. Your child must be an established patient in order for you to create an account. Please call the office to obtain a PIN number. Our Medical Records can send you an email containing the PIN to create your childs account, each child will need a PIN. Please remember, when signing up, the Initial Questions are about the Parent. Question for child(ren) will be prompted. Login to the portal on the top of our website.

What's New?

WE NOW CHECK IN WITH PHREESIA - You will be sent a text or email prior to your appointment. Please go through every prompt and asnwer all questions. Take pictures of your ID & inurance card and upload. Your child's information will be required to be verified at every visit. You may need to re-schedule your apt if not completed.
